Also, if on steam, go to properties of the game, go to Local Files, and click 'Verify Integrity of Game Cache'. Find your graphics card and re-enable it, probably not the cause for you but was my problem. The ONLY differences is that I'm using a function that has all the code in it and I rename g_pd3d and g_pd3dDevice to pd3d and pd3dDevice (without the g_) As you would disable the graphics card, launch the game and quickly re-enable before it finishes loading.
